Before your first appointment, our team can contact your insurance company to review your behavioral health and medical benefits.
To begin, please complete our Insurance Information and Benefits Verification Form and upload clear photos of the front and back of your insurance card.
The information you provide helps us review:
Whether Altium Health is in network with your plan
Your deductible and how much has been met
Copay or coinsurance amounts
Whether prior authorization is required
Coverage for the service you are considering
Any limitations or visit requirements
Your estimated financial responsibility
