How do I get my meds refilled?

For all medication refills, prescription questions and/ or pharmacy updates please go to our Current Clients page and fill out the Patient Refill Request Form.

Please allow 2 business days for refill processing. Refills are reviewed during clinic hours (Monday–Friday, 9 AM–7 PM). To avoid delays, make sure you have an active appointment on file with your prescriber as needed.

How do I ask a question about my prescription?

How can I change, cancel, or reschedule an appointment?

You can reschedule or cancel appointments in any of the following ways:

Please provide at least 24 hours’ notice for all cancellations or changes to avoid no-show fees.

💰 No-show fees: $25 for follow-ups, $50 for new appointments.

📞 Call us directly to speak with our front desk who can help you find the right provider and service.

Once your form is received, our team will reach out to verify insurance, confirm the best provider match, and schedule your first appointment.

How do I schedule as a returning patient?

Returning patients can schedule by:

💻📲 Fill out our Online Form: Visit New Client page to fill out our intake form to make sure we have the most up to date information and mark that you have been a patient with Altium before and how long ago so we know whether your treatment plan needs reevaluation
