Medical Record Requests
We understand that medical records are often needed for various reasons, and we’ve worked to improve our process based on recent feedback. If you need your medical records faxed or mailed to another provider or organization, please complete this form. Allow 3-5 business days for processing. If we don’t have a release of information on file, we may contact you via email or phone to gather the necessary details.
